Re: REQUEST TO RECONSIDER DECISION IN TERMS OF SECTION 18(1) OF SOCIAL ASSISTANCE ACT.

I, PATRICK NHLANHLA NSIBANDE, ID Number: 8012075888088, a resident of Jozini Local Municipality, under Ingwavuma DSD Service Office, would like to request the Manager of the Agency to reconsider the decision taken to reject my application, as I can prove that the end result of the assessment was not based on the actual reason of the application.

1. Doctors booking was done on the 07-12-2020
2. Third surgery @ Mosvold Hospital on the 09th February, (Still haven't seen the doctor)
3.Mid-Feb (Around 15-18) we were called to come and make no mistake, due to transport challenges I had to borrow some mobey to hired a car to take me there from EMsiyane to Ingwavuma DSD office, while it wasn't the first time as my hospital doctor's appointment same story car hire (refer to Mosvold hospital records : 801207/20), we were then informed to return back the following Thursday, of which never happened because I then received a call informing me not to come as it was stated, and should wait for another call for a new appointment date.
4. On the 12th April I was contacted for the appointment scheduled for the 15th April 2021.
5. 129 days of hard rough days I am told my application was rejected as if it was just a minor accident and be judged for current condition on a previous case application with a very different condition.

I do understand the lockdown regulations and abide by them, but in this case it has no negetive impact because this is directly linked to SASSA and the doctor poor service and turning a blind eye in this regard. This guy with no name tag just saw my nice expensive coat, boom I have been assessed then.

Some of the factors that should be considered includes:
1. The initial operation dates vs Assessment & Outcome date.
2. Initial Doctors booking date vs Assessment & outcome date.
3. The assessment tool used to assess my case, as I have the evidence to support my case.
4. I believe Temporary Disability Grant is basically for Primary cost of living, to be of temporary assistance while he/she can't manage some primary cost like food as I was on soft food, transport for medicals for almost 2 and a half months. I feel I have been deprived my Right in this matter in fact my Right was stolen.

In addition tobthe above mentioned, I would also voluntarily give you an access to my hospital files: Mosvold : 801207/20

Ngwelezana Hospital: 801207/20, and I will attach more evidence in photos for further clarity on the matter.
